Special Terms for Test Cohort (Pilot)

Provider: Cue Life DK ApS (CVR [45653110]), [address], Denmark (“Cue Life”, “we”, “us”)

Participant: The individual joining the Test Cohort (“you”, “Participant”).

Program: Cue Life Men’s Health Optimization Program (the “Program”).

Cohort Period: 3 months from your onboarding date, unless ended earlier under these Terms.

Value in Kind: Program list price approx. DKK 120,000 (≈ “16k program”), provided free of charge under these Terms.

These Special Terms apply in addition to Cue Life’s Terms & Conditions and Privacy Policy. In case of conflict, these Special Terms govern for the Cohort.

1) Purpose of the Cohort

This Cohort is a structured pilot to validate and refine our clinical and operational workflows (onboarding, blood testing and clinical presentation, check-ins, chat experience, and 3-month program outcomes). In exchange for free access, you agree to provide close, systematic feedback.

2) Eligibility

  • Age 18+ and DK resident, capable of entering a binding agreement under Danish law.
  • Willing and able to complete required labs, visits (virtual or in-person), surveys, and check-ins.
  • Your health profile must be suitable for the Program at the clinician’s discretion. We may refuse or pause participation if clinically appropriate.

3) What You Receive (Free)

  • Full access to the 3-month Program (clinical consultations, blood testing orders, clinical presentation, structured check-ins, chat support, and personalized plan), as described during onboarding.
  • Standard inclusions only. Add-ons (e.g., extra labs/imaging outside scope) may be excluded or require separate consent/fees if you opt in.

No cash alternative. The free access is non-transferable.

4) Your Commitments (the “Give-Back”)

You agree to diligently complete the following within the Cohort Period:

  1. Onboarding
    • Identity verification, medical history questionnaires, baseline PROs (patient-reported outcomes).
  2. Diagnostics & Clinical Presentation
    • Complete required blood testing within timelines we provide.
    • Attend the clinical presentation of your results.
    • Follow any pre-test instructions (e.g., fasting).
  3. Engagement & Check-Ins
    • Participate in scheduled check-ins (e.g., biweekly) and respond to chat prompts in a timely manner (typically within 48h).
    • Complete short surveys/forms after each major step (onboarding, blood test, clinical presentation, check-ins, chat/support experience, 3-month review).
  4. Feedback Quality
    • Provide specific, candid, and constructive feedback on your experience of each workflow step.
    • Report any adverse events, side effects, or safety concerns immediately.

If you repeatedly miss required steps or fail to provide feedback, we may remove you from the Cohort (see Section 10).

5) Data, Privacy, and “Near-Research-Level” Compliance

  • Your personal data will be processed per our Privacy Policy and applicable Danish/EU law (GDPR, Sundhedsloven, Journalføringsbekendtgørelsen).
  • We may use de-identified and/or pseudonymized data for quality improvement (QI), service evaluation, safety monitoring, and internal analytics. QI is part of healthcare operations—not research.
  • Activities that constitute research will only occur with appropriate ethics oversight (if required) and separate informed consent. Opting out of research will not affect your Program access in the Cohort.

6) Testimonials & Marketing (Controlled, Consent-Based)

  • We may invite you to provide a testimonial (text, audio, video) and/or to participate in case studies.
  • Cue Life will not use your identity, image, voice, or identifiable health details in marketing without your explicit written consent (separate document or in-app flow).
  • If you consent to a testimonial: you grant Cue Life a non-exclusive, royalty-free license to use it for marketing in agreed formats/channels, for an agreed period, subject to any restrictions you select (e.g., first name only; no lab values; approval right on final copy). You may withdraw consent prospectively at any time; prior uses remain lawful.
  • You may choose a fully anonymized or initials-only testimonial. We will respect any reasonable redactions you request.

7) IP & Confidentiality (Pilot Materials)

  • All Program content, workflows, and materials are Cue Life’s confidential information and intellectual property.
  • You agree not to disclose, copy, or distribute proprietary materials or insights from the Cohort, except for your personal use. This survives Cohort end.

8) Clinical Standards & Safety

  • Care is provided by licensed clinicians under Autorisationsloven, Sundhedsloven, and applicable guidance.
  • Some interventions require labs or monitoring before initiation/continuation; clinicians may withhold or modify interventions at their discretion.
  • The Program does not replace emergency care. For emergencies, call 112.

9) No Fees; Taxes

  • You pay no fee for standard Cohort services.
  • If any benefit triggers personal tax consequences, you are responsible for your own tax reporting.

10) Suspension / Removal from Cohort

We may suspend or remove you if you:

(a) repeatedly miss required steps, check-ins, or surveys;

(b) provide materially false information;

(c) engage in abusive conduct;

(d) create safety risk; or

(e) breach these Terms or law.

We may also pause or terminate the Cohort for operational or regulatory reasons. Medical records will be retained per law.

11) Warranties; Liability

  • Services are delivered with reasonable professional skill and care. No specific outcomes are guaranteed.
  • To the maximum extent permitted by law, we are not liable for indirect, incidental, or consequential losses. Where liability cannot be excluded, it is limited to DKK 10,000 per Participant. Nothing limits liability for intent or gross negligence, or where otherwise unlawful to limit.

12) Term; End of Cohort

These Terms apply from acceptance until the earlier of: (i) 3 months after onboarding; (ii) your removal or withdrawal; or (iii) Cohort termination. Sections 5–8 and 11–14 survive.

13) Changes

We may update these Special Terms for legal, safety, or operational reasons. Material changes will be notified in-app or by email and take effect on the stated date.

14) Governing Law; Venue

Danish law governs (conflict rules excluded). Disputes are subject to the Danish courts. Consumer mandatory rights remain unaffected.
